How to begin our program
- Have your primary care physician fax our referral form to 602.798.0775.
- Please fill out and bring the patient assessment form to your first class.
The Diabetes Center will call you to schedule diabetes classes. Please allow 48 hours for your physician to send the referral form.
If you are new to diabetes and/or you do not have a glucose meter, you will have the opportunity to attend our Getting Started: Meals, Meds, and Monitoring Class prior to the diabetes class series of your choice.
